Aloha Care Connections LLC provide services as follows: 

1. Application, eligibility, admission, waitlisting, suspension, discharge, and transfer of patients 

2. Confidentiality and client rights policy and procedures.  

3. Development of service plans which includes addressed problems and needs of the clients, measurable goals, nursing interventions, and services identified specific to the client.  

4. Development and authorization of service plans. 

5. Service coordination of which includes arranging and identifying services for the client, preparing caregivers with the necessary forms and skills to implement service plan, arranging clients with care-appropriate caregivers, and conducting caregiver training for service plan skills.  

6. Service monitoring of which includes monitoring services rendered according to the client needs and service plan, continued monitoring to ensure the client is achieve goals and outcomes, nurse monthly face-to-face contact with clients, ongoing evaluation of caregiver’s services and home environment suitable to the client,  

7. Reassessment and continued follow up of service plans. 

8. Record and documentation for clients. 

9. Budgetary procedures for clients.
